Fixes and improvements:

Added several columns to be available in the Active Cases list on the Investigator’s dashboard.
Added tallies of investigative dimensions to the PHS Concerns report. These tallies used to be in the report but this report was completely reconstructed to meet certain requirements and the tallies were inadvertently left off.
